Primary Care
Health services that cover a range of prevention, wellness, and treatment for common illnesses. Our primary care providers include doctors, nurses, nurse practitioners, and physician assistants. We maintain long-term relationships with you and advise and treat you on a range of health related issues. We may also coordinate your care with specialists.
Injury Care
We provide a complete line of medical services to ensure a safe and sound recovery. The services we offer cover personalized care before, during and after injury. Our in-house providers provide periodic comprehensive examinations and co-ordination of care. We also offer evaluation, diagnosis and treatment of a wide range of orthopedic issues.
Senior Care
Our team of geriatric specialists helps you manage your changing health needs as you age. We offer comprehensive primary care and consultations to support patients and families coping with challenging conditions related to aging. Our services include primary care for adults 65 years and older who have neurological conditions such as dementia and cognitive impairment.

Managing your medications at SEAK Health is simple, from start to finish. We believe in a holistic approach to healthcare. We try and optimize diet, exercise and mindfulness to keep you healthy. However, sometimes prescription medications are required and we use evidence based medicine to support our choices. You can easily refill a current prescription by contacting your pharmacy to request a refill. Refill requests usually will require an in office patient visit to make sure that you are receiving the best care. We also offer free (with most insurances) flu shots, and enable you to make sure you have all your immunizations in order through our quick and easy questionnaire.
